Can the anion gap be too low?

Yes — an anion gap below 8 mEq/L is uncommon but real. The most frequent cause is hypoalbuminemia; rarer causes include paraproteinemia (myeloma), bromide toxicity, and laboratory error. Often it is clinically insignificant — but occasionally it is the first clue to a plasma-cell dyscrasia.

A low anion gap — conventionally AG < 8 mEq/L, or below the lower bound of your lab's reference interval — is much rarer than a high one. The decreased anion gap meaning is the inverse of the high-gap story: instead of unmeasured anions accumulating, the gap shrinks when unmeasured anions fall (chiefly albumin) or when unmeasured cations rise. Because the gap is mathematically the difference between two large measured numbers, a low value is also the pattern most often caused by laboratory error — a mis-measured sodium, chloride, or bicarbonate can drive a falsely low (or even negative) gap. The first move on seeing a low gap is to repeat the basic metabolic panel on a fresh specimen.

Why the gap can fall

The gap exists because electroneutrality is preserved: sodium is balanced partly by measured chloride and bicarbonate, and partly by unmeasured ions — mostly the negatively charged protein albumin. A low anion gap arises from four mechanisms:

  • Hypoalbuminemia (most common). Each 1 g/dL fall in albumin lowers the expected normal gap by roughly 2.5 mEq/L (Figge et al., Critical Care Medicine). A cirrhotic or nephrotic patient with albumin 2.0 g/dL has a physiologic baseline gap near 4, not 12 — so a "low" gap here is simply the expected consequence of low protein, not disease.
  • Paraproteinemia. Monoclonal immunoglobulins (IgG in multiple myeloma) carry a net positive charge at physiological pH, raising unmeasured cations and narrowing the gap (Kraut & Madias, StatPearls 'Serum Anion Gap'). A persistent, unexplained low or negative anion gap is a recognised red flag for myeloma and warrants serum protein electrophoresis and free-light-chain testing.
  • Bromide toxicity. Bromide is measured as chloride by some analyzers but at a higher equivalence, falsely elevating measured chloride and shrinking — sometimes inverting — the gap.
  • Hypercalcemia, hypermagnesemia, lithium toxicity. These raise unmeasured cations and narrow the gap, though rarely enough to push it below 8 alone.

Clinical significance

Most low gaps are clinically insignificant — they reflect hypoalbuminemia, the commonest cause, and resolve as the albumin recovers. The reason not to dismiss a low gap entirely is the paraproteinemia differential: an unexplained, persistent low (or negative) anion gap can be the earliest laboratory clue to multiple myeloma and warrants a myeloma workup.

What to do with a low gap

First, repeat the panel on a fresh draw to exclude lab error. Next, check the albumin — if it is low, the low gap is expected and usually needs no further workup beyond correcting the underlying cause. If the albumin is normal and the gap is persistently low, send serum protein electrophoresis (SPEP), urine protein electrophoresis (UPEP), and serum free light chains to screen for a monoclonal gammopathy.
